The Primex³ Health Risk Appraisal (HRA) is a simple online, confidential series of questions to determine your personal health risks. Once finished, you will be provided with an individualized report that outlines your health risks, that if not addressed, could leave you susceptible to developing a chronic health condition. You are encouraged to share this report with your primary care physician. Your employer will not receive a copy of your personal report.

To take the Health Risk Appraisal, you must create an HPHConnect account using your member number on your Harvard Pilgrim Identification card. If you already have an HPHConnect account set up, you may take the HRA at any time. If you do not have an account, you will be prompted to create an HPHConnect account the first time you visit the page below.

Through your HPHConnect, you can also view health records, account information, or get health-related support. The "Personal Health Records" feature has tools that let members see their medical history, office visit summary, illness/conditions history, family history and more. Members can give permission to providers to view the reports or print them out to take to office visits.
